The Exceptional Access Program (EAP) facilitates patient access to drugs not funded on the Ontario Drug Benefit (ODB) Formulary, or where no listed alternative is available. The new online service (to be called The Special Authorization Digital Information Exchange [SADIE]) will deliver two core advancements in the EAP service: 1) an online digital service for physicians to manage EAP requests; and 2) automation of the many currently manual back office processes.

The online service will create a digital channel for prescribers to access the EAP program, research drug criteria and availability, create, submit, and manage requests, receive notifications and alerts from the ministry for tasks such as responding to requests for additional information, submitting renewals, etc. The introduction of automation to many manual processes, including to the adjudication process itself, will provide real-time, automated responses for many drug requests ensuring that patients receive timely decisions on funding for drugs.

Come join representatives from the Ministry of Health & Long-Term Care to learn more about SADIE, experience what the solution will look like, and share your thoughts on EAP which can contribute to the design of the new solution

As clinicians, we experience care coordination issues daily. For example, when our complex care patients require multiple members of the care team; or when high needs patients have visited the ED several times in a month; or when a patient’s family is struggling to care for them at home. For care to be better coordinated, we know that it needs to be integrated and centered on the patient. It is simply not possible to coordinate care without the necessary integrated clinical information across the continuum to ensure effective communication amongst all members of the patient's care team.

Technology can help us address this much-needed integration. Integrating EMRs with other provincial IT tools empowers clinicians and patients with an ever-improving range of information available no matter the care setting. In addition, tools like HRM (Health Report Manager) and e-notifications help to deliver tangible value to clinicians and patients. As a near real-time electronic message sent through HRM to primary care providers, e-notifications notifies them when patients are discharged from ED or admitted or discharged from in-patient units. Coordination is improved across the highly critical boundary between primary and secondary care.

This session aims to introduce the audience to how HRM and e-notifications assists with care coordination; the approaches and benefits from EMR integration and technologies now available in Ontario.
