Closing the Gap: Using EMRs to improve health equity
In this opening keynote Dr. Tara Kiran will address how family physicians can consider collecting an d using data in their EMR to address the social determinants of health.

About Dr. Tara Kiran:
Dr. Tara Kiran is a family physician and clinician scientist in the St. Michael’s Hospital Academic Family Health Team and Department of Family and Community Medicine, University of Toronto. She is an Associate Scientist at the Centre for Urban Health Solutions, St. Michael’s Hospital and an Adjunct Scientist at the Institute for Clinical Evaluative Sciences. In 2015, she was awarded a New Investigator Award from the North American Primary Care Research Group for her program of research evaluating the impact of Ontario’s primary care reforms. Dr. Kiran has led her Family Health Team’s Quality Improvement program for the last six years and is actively working to understand and improve quality of primary care. As of October 2016, she has taken on a new role as an Embedded Clinician Researcher with Health Quality Ontario where she is developing a program of research to improve care for patients transitioning from hospital to home.

This presentation will demonstrate how the use of order sets, documentation templates, automated decision support, reporting tools, and patient portals can be used to improve mental health care delivery. A particular focus will be paid to improving process efficiencies and quality of care.
