New EMR Adoption Program Extension 2

The New EMR Adoption Program Extension 2 provides physicians with funding to transition from paper-based records to a certified electronic medical record system.

EMR funding includes both one-time payments and monthly subsidies to help physicians with the associated costs incurred over a 3-year period. Please see the funding table below for payment details.

  1. Go Live Declaration - A participating physician must submit a Go Live Declaration indicating that the EMR has been implemented within ten (10) months of signing the Funding Agreement to receive payment for this milestone. Where Go Live requirements have not been met due to reasons beyond the control of the physician (e.g., vendor-related issues) and where an exemption has been granted in writing, the Go Live one-time payment may be permitted. In any case, monthly payments will not be made after October 31, 2018.

  2. Performance Declaration and EMR User Survey - A participating physician must submit a Performance Declaration and an EMR User Survey within twenty-two (22) months of signing the Funding Agreement to receive the full payment for this milestone. Where Performance Recognition requirements have not been met due to reasons beyond the control of the physician (e.g., vendor-related issues) and where an exemption has been granted in writing, the Performance Recognition payment may be permitted.

EMR Adoption Program - Electronic Funds Transfer (EFT) Form
The Electronic Funds Transfer (EFT) form is submitted along with a void cheque at the Funding Agreement stage. The name on the void cheque must match the name of the applicant.

New EMR Adoption Program Extension 2 - Vendor Contract Declaration
The Vendor Contract Declaration (VCD) confirms that the applicant has signed a contract with an EMR vendor for a certified EMR product and related services.

New EMR Adoption Program Extension 2 - Go Live Declaration
The Go Live Declaration (GLD) is submitted within ten (10) months of signing the Funding Agreement. The applicant must complete acceptance testing of the EMR and start to use it.

New EMR Adoption Program Extension 2 - Performance Recognition Declaration
The Performance Recognition Declaration (PRD) form is submitted when a family physician has established an EMR for the lesser of two thirds or six hundred (600) of his or her rostered or active patients. The EMR User Survey (completed online) accompanies the PRD.

New EMR Adoption Program Extension 2 - Funding Agreement
The Funding Agreement is completed and signed only after a Practice Management Consultant has completed a Clinic Information Form. Once a Funding Agreement is signed by OntarioMD and the applicant(s), funding is reserved for the signatories.

EMR Adoption Program - Notice of Change Form
The Notice of Change Form is submitted to OntarioMD when there is a change in the applicant's EMR usage, contact information, physician participation (departures, replacements and additions), applicant restructuring and practice closure.

EMR Adoption Program - Upgrade Notice of Change Form
Use this form to notify OntarioMD of changes that could affect the group's participation in the EMR Adoption Program. This includes changes in the group's EMR (offering, version, and specification) and changes in the participating physicians in the group.

EMR Adoption Program - Non Participating Physician Consent Form
The Non-Participating Physician Consent Form is to be completed by a group's Lead Physician to provide the names of physicians who are not participating in the New EMR Adoption Program Extension 2.


 

EMR Enhanced Use Program

The EMR Enhanced Use Program provided eligible physicians with a monthly funding subsidy to offset the cost of maintaining their EMR, including upgrading to Ontario's latest EMR Specification. The EMR Enhanced Use Program was available from April 1, 2014 to March 31, 2015 and replaced the EMR Product Upgrade Program. EMR Enhanced Use Program - Funding Agreement
