HRM / Fax pilot project for pathology results

Lumeo hospitals recently piloted pathology result distribution via HRM® (Health Report Manager) from six hospitals in southeast Ontario to a select group of primary care clinicians. As part of the second phase, we are now expanding the pilot to include all primary care clinicians, referring clinicians, and copied-to (CC'd) clinicians using HRM.

To support a smooth transition and ensure the accurate delivery of results, reports will be sent via both fax and HRM for approximately one month starting on June 10, 2026. This parallel delivery period will help to ensure that reports are received correctly through HRM before fax delivery is discontinued.

During this time, if you experience any of the following issues, please let us know using the Lumeo HRM / Pathology Pilot Feedback – Fill out form so that we can investigate the issue:

  1. Missing reports — received via fax, but not through HRM

  2. Duplicate reports — received more than one copy of the same report through one of the delivery paths (either duplicated HRM results or fax results)

  3. Content mismatches — differences between faxed results and HRM-delivered results

Note: You should only receive reports once (via fax/HRM), even if you are mentioned twice (e.g., as primary care and referring).

Upon successful completion of the pilot and validation of HRM report delivery, fax distribution will be discontinued for clinicians receiving pathology results through HRM. We will send you further communications regarding timelines and the process for discontinuing fax delivery before we implement any changes.

Thank you for participating and helping us improve our system. For more information about the Lumeo collaboration with HRM and OntarioMD, read the article, Tackling a wicked problem through HRM optimization.
