Sending Facilities Live on HRM®
Notice of Security Incident at Waterloo Regional Health Network
OntarioMD and Ontario Health detected a security incident involving Health Report Manager (HRM®) and affecting the Waterloo Regional Health Network (WRHN). WRHN sends patient reports to clinicians in the community via HRM. This issue was immediately contained upon discovery.
This incident did not impact any other hospital organizations.

Sending Facilities Overview
HRM sending facilities include hospitals and specialty clinics across Ontario. Sending facilities are generally listed by individual site if they contribute to HRM from multiple locations (e.g., Niagara Health System). However, in some cases, they are listed as one facility, the hospital or clinic corporation (e.g., The Ottawa Hospital), sending reports from multiple locations, respectively, through HRM.
Sending facilities will, at times, update the report types they send through HRM. Please check regularly to ensure that you have the latest versions of the Report Type lists (indicated in red).
How to stop paper fax reports
Click the plus (+) icon next to the facility that is sending you faxed reports. This will open the facility’s contact information. Send an email to the Support / Stop Paper Contact email address listed and request that they stop sending faxed reports to your clinic.
You can use the email template below to request that paper/fax reports be stopped (This template is also available as a Word document that you can copy and paste into an email).

Subject: Request to Stop Duplicate Fax Reports
Hello [Facility Name],
OntarioMD has identified you as the appropriate contact for stopping faxed or paper reports from your facility.
Our clinic is currently receiving reports from your facility by both fax and Health Report Manager (HRM®). We kindly request that you send reports via HRM only and discontinue sending reports by fax.

How to report a privacy issue to a hospital:
Click the plus (+) icon next to the facility that sent the report with the privacy concern (for example, if you received a report that wasn’t meant for you). This will show the facility’s contact details. Send an email to the Privacy Contact email address listed and explain the issue.
What if you can’t find the contact information for the sending facilities, or it’s outdated?
If the sending facility’s contact information is missing, outdated, or not working, please contact Support@OntarioMD.com for help.
Please note that OntarioMD is in the process of updating the sending facilities list from Local Health Integration Networks (LHINs) to Ontario Health Regions.
