Following our wildly successful Digital Health and Virtual Care Day, OntarioMD is extremely pleased to begin 2021 with a 10-part series of online learning, OMD Educates: Digital Health and Virtual Care Curriculum. The sessions will be conducted by OntarioMD Peer Leaders and experts with a focus on education and clinical enhancements using digital health and virtual care tools.

Targeting Patient Populations in OHTs

Ontario Health Teams (OHTs) were created to address the many frustrations primary care clinicians have every day with a fragmented health care system and the impact on their patients. By joining an OHT, primary care clinicians become part of a team that helps reduce frustration.

OHTs can provide better coordinated care for patients, smoother transitions of care, share vital health information, and help develop and improve care pathways in their local communities that form the OHT. Get tips from OMD Peer Leaders on February 23, 2022, on how they are leveraging their local OHTs to target their local patient population.

This Group Learning program has been certified by the College of Family Physicians of Canada for up to 2 Mainpro+ credits.

Physicians must complete the evaluation to receive their CME certificate. At the end of the curriculum participants should have a better understanding and knowledge of digital health and virtual care tools and how to best implement to benefit their practice and patient care.

This event is an Accredited Group Learning Activity (Section 1) as defined by the Maintenance of Certification Program of the Royal College of Physicians and Surgeons of Canadaand approved by the SOGC. You may claim a maximum of 8 hours (credits are automatically calculated) following the completion of the entire OMD Educates Digital Health and Virtual Care course in October, 2021. It is important that you fill out the evaluation (link provided during session) for each session to confirm attendance and receive your certificate.
