What if patients really were first?
Symposium Description: Transitions along the continuum of care (as a patient moves between caregivers and between different health care settings) are complex. In an increasingly technology-enabled health care system where technology is supposed to improve the patient experience, these transitions in care remain challenging. This panel discussion will address the complexity of transitions between caregivers and multiple settings. There will be a focus on roles and responsibilities in the health care system, how to improve quality in communications, and the place of technology in the patient's journey so that transitions become seamless. The ability of new technology systems to transfer personal health information in order to enhance patient care during transitions will also be highlighted.Speakers:
